Healthcare Insurance

All eligible employees can enrol in the healthcare insurance with Lux Med. The company provides employee cover and you can choose to add your dependants at your own cost.

This is a comprehensive plan covering outpatient, dental, diagnostic, rehabilitation, hospital and long term care. Details of the cover can be found in the following plan scope:

If you would like to add your dependants to cover you can do so at your own cost shown in the table below:

How to enrol?

You are required to enrol in plan by the 20th of the month for cover effective from 1st of the following month. Please note that if you decide to add your dependants that this is for a 12 month period.

Enrolling in the insurance is a two- part process:

  1. Create an account
  2. Complete the declaration

  1. In order to create an account in the e-declaration system visit this website enter the access code BRAN7345 and press “enter”
  2. You will be redirected to the website for user account registration
  3. Create a login and enter a password which you will be using for the application. All fields marked with a * are mandatory.
  4. Complete the remaining required data. If you are a Polish citizen enter your PESEL number in the number field. If you are a citizen of a different country, set your citizenship choose one of the document types available on the list and enter the series and number of your document in the number field.
  5. Accept the terms and conditions and the privacy policy and sign up for the newsletter if interested.
  6. Click on “Register” at the bottom of the page. You have now completed the account creation process. You will be redirected to complete your e-declaration.

  1. Click on “Fill out the declaration” on the left side of the screen
  2. Select the offer that you would like to join
  3. Complete the mandatory fields marked with a * and then:
    • If you would like to join the insurance as an individual (without any dependants) click on “Go to summary”, If the information is correct then click “save”. If you would like to correct it change any declaration details, click on “Go back” and change the selection or your information in the declaration, then go to the summary and save.
    • If you would like to add dependants (spouse, partner and or child) click “Employee’s partner” or Employee’s child” and select the option you wish to choose. Complete the mandatory fields with a *, If your family member has the same address as you, you can press “autofill address”. After completing this go to summary. If the data in the summary is correct click “save”
  4. Accept the terms of joining and mark the required declarations.

Adult dependants will be sent a link with a request to accept the joining of the insurance and mark the required declarations.

For children, the parent or legal guardian accepts this on their behalf by clicking on the link sent to the email address in the e-declaration.

Using your healthcare plan

Lux Med provide a patient portal where you can easily:

  • make and cancel an appointment,
  • check laboratory test results,
  • order a prescription,
  • use the Online Consultation system,
  • gain access to your medical history,
  • make an appointment for occupational,
  • medicine examinations.

You can register for this here and complete the registration form or you download the Patient Portal App, available for devices with iOS and Android operating systems.

Once you have registered you need to confirm your identity by:

  • visiting the Patient Portal (if you are 18 or over),
  • calling the Phone Line on 22 33 22 888,
  • completing the contact form here,
  • at a facility,
  • electronic banking: mojeID.

Multi Sports Card

At Cision we believe in supporting the health and wellbeing of our employees. By offering a multi sports card benefit, we aim to promote and support for your physical and financial wellbeing.

You will have the option of selecting from three types of membership:

  • MultiSport Plus – multiple entry to sports facilities each day, with more than 5,200 sports facilities available.
  • MultiSport Classic – access one sports facility per day and have access to over 3,200 sports facilities available.
  • MultiSport Light – to be used according to the number of entries recorded on a card and no more than once per day with more than 3,100 facilities available.

How to enrol?

Vision Allowance

Employers are required to provide employees with corrective glasses or contact lenses, in accordance with a doctor’s recommendation, if eye examinations conducted as part of preventive health care indicate the need for their use while working with a computer monitor.

If you require glasses or contact lenses for working at a computer monitor then you can request a reimbursement of up to 500 ZL per year.
